Broken Form?

Energy Apps for Ontario Resource Sign-Up Form doesn't allow 'submit' of the form.

    Actually, I found a bug when trying again! If one doesn't specify a required field, the form is redisplayed with NO submit button! This is basic programming error. Let me know if you need help to fix the form (which should be very easy for an average programmer).
