Register an App by Third Party BROKEN

For some reason when the form is submitted it is giving a 500 internal error.

Is the form broken?

    Problem still seems to exist. Here is the url im using:
    https://greenbutton.affsys.com/auth/mvc/app?action=add

    and error:
    An error occurred while registering application:Internal Server Error [500]

    I'm also getting a 500 error on the registering an app with the Data Custodian ... any update on this?
